Job summary

Keystone Health is seeking a Care Manager (RN) to provide patient-centered care coordination, helping patients navigate the healthcare continuum while improving health outcomes and patient experience. This full-time role focuses on chronic disease management, care planning, care transitions, closing care gaps, and connecting patients with community resources.

The Care Manager also addresses Social Drivers of Health (SDOH) and utilizes the SBIRT model to support patients with substance use

Qualifications

• Associate degree in Nursing required; Bachelor's degree preferred.
• Current RN license and CPR certification required.
• Clinical nursing and care management experience preferred.

Responsibilities

• Coordinate care plans and support patient care transitions.
• Educate patients and families to support self-management.
• Identify care gaps and barriers to care.
• Connect patients with community resources and support services.
• Collaborate with providers, care teams, and community partners.
• Utilize SBIRT practices to support patients with substance use concerns.
