Job summary

Catholic Health Initiatives is seeking a RN Care Coordinator to lead discharge planning and coordinate care across the patient journey. The role emphasizes collaboration with physicians, nursing, insurers, and post-acute providers to ensure timely, high-quality transitions and optimal patient outcomes.

The position requires an RN license in Texas, at least two years of acute hospital experience, and BLS/CPR readiness.

Qualifications

• Must be a graduate of an accredited school of nursing with minimum two years of acute hospital clinical experience.
• TX RN license required; CPR/BLS within 90 days of hire; Master’s in Case Management or Nursing and 1 year of experience preferred.

Responsibilities

• Completes and documents discharge planning assessments using the screening process or on request; reassesses and updates plans as needed.
• Facilitates development of a multidisciplinary discharge plan with patient, family, and post-acute providers.
• Oversees and evaluates implementation of the discharge plan.
• Collaborates with multidisciplinary team to ensure progression of care and appropriate inpatient resource use.
• Coordinates with post-acute providers to ensure timely transitions to appropriate post-acute settings.
• Identifies readmission risk and implements mitigation strategies for at least 30 days.
