Job summary

Prosano Health Solutions, in partnership with Blue Cross Blue Shield of Arizona, seeks a Clinical Care Guide (RN) to join our on-site care team in Arizona. You will manage specialist and diagnostic referrals, support patients in-person and virtually, and drive care excellence while protecting privacy and compliance.

Ideal candidates hold an RN license in AZ, an associate degree in nursing, BLS, and EMR experience.

Qualifications

  • Current state license to practice as a Registered Nurse.
  • Associate's degree in nursing required.
  • BLS certification required.
  • EMR experience and customer service orientation.

Responsibilities

  • Provide in-person and virtual support with real-time consultation and scheduling of referrals.
  • Contribute to improved health outcomes and cost savings.
  • Explain referrals for specialist and diagnostic services clearly.
  • Manage end-to-end referral and service processes.
  • Lead quality programs and patient outreach to gaps in care.
