You will coordinate patient care plans, analyze EHR data to support decisions, and help ensure safe, efficient telehealth transitions throughout the patient journey.

You will educate staff on new workflows, promote adoption of digital tools, and maintain quality, safety, and compliance standards while collaborating with clinicians and technologists.

Qualifications

• Active RN license in Washington or eligible to transfer.
• Strong care coordination experience required.
• Familiarity with EHR systems and data-driven care decisions.
