Qualifications

• Minimum of three years RN experience or equivalent with relevant patient population
• BSN required
• Effective verbal and written communication skills required
• Ability to prioritize and make independent clinical judgments
• Experience with basic patient care equipment and protocol manuals
• Ability to collaborate with an interdisciplinary team
• Strong critical thinking and ability to evaluate care across the continuum
• Nebraska RN license or compact state license
• BLS certification from an AHA endorsed program

Responsibilities

• Coordinate a patient care delivery system for designated patients to achieve high quality, cost-effective outcomes
• Perform duties in ambulatory settings and assist with transitions of care across multiple settings
• Provide targeted interventions to avoid hospitalization and ER visits
• Support care management within primary or specialty care as assigned
