Job summary

Mount Carmel Health System is seeking an RN Care Navigator to coordinate and deliver patient care in a primary care setting. This role adheres to the Ohio Nurse Practice Act and participates in a team-based environment.

The position is based in Mount Carmel Medical Group locations in Columbus area, with typical office hours from Monday to Friday. The role requires nursing licensure in Ohio and prior ambulatory/care management experience.

  • Ambulatory care experience preferred (2+ years)
  • Case/care management experience preferred (1 year)

Description:

Position Purpose: RN Care Navigator, within the scope of the Ohio Nurse Practice Act, will provide care in an atmosphere sensitive to each person's physical, emotional, social and spiritual needs. The Registered Nurse Care Navigator, functioning in the spirit of teamwork, is accountable for the coordination and delivery of patient care utilizing the Nursing Process within the standards, policies, procedures and guidelines of the Organization.

Hours of office: Monday - Friday 8:00am - 4:30pm

Minimum Qualifications:
  • Education: Graduate of a school of nursing. Associate's Degree required Bachelor's preferred.
  • Licensure / Certification: Registered Nurse; current, unrestricted license to practice in the State of Ohio.
  • Experience: Minimum of two years experience in an ambulatory setting preferred.
  • One year Case / Care Management experience preferred.
