Qualifications

• Associate Degree or higher preferred.
• 1-2 years of related experience in the profession required.

Responsibilities

• Oversee daily operations of the Care Management team, ensuring staffing and performance.
• Provide mentorship and support to Care Managers.
• Coordinate collaboration among physicians, nurses, social workers and other professionals.
• Monitor metrics like length of stay and readmission rates and implement improvements.
• Lead staff orientation, training and competency development.
