Job summary

Cityblock Health, Inc. in Baltimore is seeking an RN Care Manager for a predominantly telephonic role (about 90%), with occasional field visits.

You will manage a panel of clinically complex members, create and monitor care plans, and collaborate with the Cityblock Care Team and external providers to improve outcomes. The role requires an RN with at least 3 years of experience, strong clinical judgment, and expertise in behavioral health screening (PHQ-9, GAD-7, AUDIT, DAST-10).

Qualifications

• Graduate of an accredited school of nursing (RN).
• Minimum 3 years of nursing experience.
• Demonstrates strong clinical judgment and communication skills.
• Proficiency using electronic health records and scheduling tools.

Responsibilities

• Manages a panel of clinically complex members and develops care plans.
• Performs assessments with PHQ-9, GAD-7, AUDIT, DAST-10 screeners.
• Conducts in-person visits when needed and coordinates care with teams.
• Documents care plans, tracks progress, and communicates with partners.
• Reconciles medications and supports transitions of care.
