Schedule: Full Time

Shift: Monday-Friday, 8AM-5PM

Location: Remote, with in-person meetings in Maitland, FL. Must reside within 50 miles.

The role you’ll contribute:

The Nurse Health Advisor coordinates care, educates clients, and provides interventions within the scope of case management practice, directly influencing clinical and financial outcomes. It involves a systemic collaborative approach to providing and coordinating healthcare services across the continuum, including identifying high-risk clients, assessing needs, collaborating with providers, and developing care plans with accurate documentation. The role aims to reduce fragmentation and duplication of care, enhance quality, and promote cost-effective outcomes, while maintaining outstanding customer service and respectful relationships.

  • Identify high-risk clients, monitor, assess, and evaluate healthcare and psychosocial needs; collaborate and coordinate care and services between healthcare providers.
  • Develop and implement care plans with immediate and long-term goals; document interventions and activities aligned with care management objectives.
  • Adhere to case management standards and evidence-based clinical guidelines; incorporate whole person care using the CREATION Health model to promote quality and cost-effective outcomes that support physical, psychosocial, and vocational health.
  • Assess clients' activation stage, literacy, and self-management capabilities; refer to appropriate team members or community resources; incorporate assessments into care plans.
  • Facilitate advanced care planning and collaborate with behavioral health to prevent unnecessary hospitalizations and readmissions.
  • Educate clients and families on diagnoses, medications, nutrition, and self-management strategies.
  • Monitor and document responses to interventions and client progress, utilizing motivational interviewing to foster positive behavior change and achieve goals.
  • Collaborate with the interdisciplinary team to develop person-centered care plans that prevent avoidable hospitalizations and ER visits.

Qualifications:

  • Graduate of an accredited nursing program
  • 5+ years of clinical nursing experience
  • Current registration with the Florida State Board of Nursing
  • Valid Florida RN license

Preferred qualifications:

  • Bachelor's or higher degree in Science or related field
  • Experience with telecommuting and related metrics
  • Minimum 5 years’ experience in case management, quality assurance, utilization management, insurance/managed care, hospice, home health, ED/ICU, or geriatric care
  • At least 1 year of experience with virtual meetings using platforms like WebEx, Skype, FaceTime, Go-To-Meeting, or Zoom
