Premise Health is seeking a Full Time Care Manager RN to provide accessible, comprehensive primary care for Hormel Foods employees and dependents at our Austin, MN Health Center.

The role requires an active RN license, BLS/CPR, and 3+ years of experience in chronic condition management. Epic experience preferred. We offer a comprehensive benefits package and opportunities to advance in a multidisciplinary team setting.

Qualifications

• Licensed RN in the state of practice.
• Current BLS/CPR certification.
• BSN preferred; 3+ years RN experience in chronic condition management.

Responsibilities

• Develop personalized treatment plans using provider plans and Lifestyle Medicine.
• Coordinate with multidisciplinary team to promote prevention and education.
• Document touchpoints in the EMR and track progress.
