• Execute standing orders for tests and preventive services.
• Coordinate referrals to specialists, hospitalizations, and other services; provide timely telephone advice as per protocol.
• Prioritize care management activities to achieve optimal quality and cost outcomes.
• Collaborate with the care team to develop and implement individualized care plans and SMART goals.
