Corewell Health's Butterworth Hospital in Grand Rapids is seeking a full-time Registered Nurse to plan and coordinate patient care, manage a caseload, and collaborate with the health team to optimize outcomes across the continuum of care. The role emphasizes education, care coordination, and data-driven practice.

Ideal candidates will have a Bachelor’s degree in nursing or a related field, Michigan RN licensure, and BLS certification at hire.

Qualifications

• Bachelor's Degree or equivalent nursing or healthcare related field.
• 2 years of relevant clinical nursing and/or case management experience required.
• 5 years of relevant experience preferred.
• Registered Nurse (RN) - State of Michigan Upon Hire required.
• BLS certification required (AHA or ARC) Upon Hire.

Responsibilities

• Plans and prioritizes care for individuals and populations to promote optimal health.
• Develops care plans and coordinates with PCPs, specialists, and the health team.
• Assesses educational needs and implements teaching strategies.
• Coordinates services with patients, providers, and payers to improve access and outcomes.
• Documents data, plans, interventions and outcomes per department guidelines.
