Job summary

The Center for Comprehensive Care & Diagnosis of Inherited Blood Disorders (CIBD) is seeking a Care Manager RN to coordinate patient care across the continuum within a team-based model. This on-site role emphasizes collaboration with physicians, nurses, and external providers to optimize outcomes.

Responsibilities include program development, community engagement, and strong documentation in the EMR. CA RN licensure, BLS, and HIPAA training are required within a set timeframe.

Qualifications

• Active California State RN license.
• BSN preferred for advancement and program development.
• On-site work required and HIPAA compliance.

Responsibilities

• Coordinate care with patients, internal team and external facilities (home health, labs, pharmacies).
• Collaborate with physicians, nurses, and coordinators to ensure seamless care and fulfill orders from start to finish.
• Document effectively in EMR and communicate via email/Zoom/Microsoft Teams with patients and colleagues.
• Support program development and community outreach through the care management team.
