Job summary

Responsibilities

• Coordinate continuity of patient care with patients and families during admission, discharge, ER visits, and PCP visits.
• Manage high-risk patient care and patients with comorbidities or high readmission risk.
• Provide education to parents/families after admission and coordinate discharge planning.
• Serve as a resource for the team to promote advocacy for patients, families, and Arkansas Children's team.
• Conduct comprehensive preventive screenings for patients and assist staff in daily patient interactions.
