You will lead intensive care management for adults with complex needs, acting as a key liaison among physicians, social workers, therapists and the care team to optimize discharge planning and length of stay.

Qualifications

• Bachelor's degree in Nursing (BSN) and NYS RN licensure required.
• 3–5 years of acute care nursing experience.
• Spanish language fluency preferred.

Responsibilities

• Provide intensive care management for adults with complex medical needs.
• Coordinate with interdisciplinary team to plan care and discharge.
• Develop individualized care plans to optimize length of stay and prevent readmission.
