Job summary

Premier Health Partners in Dayton, OH is seeking an RN Care Manager to organize and coordinate care across the care team. You will develop individualized care plans, monitor progress, and use registries to trigger interventions, striving for patient-centered outcomes.

You will lead communications with the care team and patients, perform ongoing assessments, provide education outside routine visits, and foster strong professional relationships with patients and families. EPIC experience preferred.

Qualifications

• RN licensure current in Ohio; BSN required.
• Minimum three years nursing experience; leadership preferred.
• Solid knowledge of chronic conditions and patient assessment.
• Accurate data handling and computer skills; strong communication.
• Self-directed, able to work independently and multi-task.

Responsibilities

• Document mutually agreed care plans addressing patient goals and risks.
• Perform ongoing assessment, monitoring, and tracking in the EHR.
• Provide proactive care and education outside routine visits (phone/portal).
• Build trusting relationships with patients, families, and caregivers.
• Utilize self-management tools and support shared decision-making.
