Qualifications

• Bachelor's degree required; RN licensure in Ohio.
• Current unencumbered RN or APP-RN licensure in Ohio.
• 2–5 years nursing practice experience in acute hospital, community or public health nursing concepts.

Responsibilities

• Coordinate care with patients, families, caregivers and providers to deliver organized, cost-effective and patient-centered care.
• Collaborate with interdisciplinary staff to promote patients’ optimal health through disease prevention and management strategies.
• Apply evidence-based interventions to populations to support cost-efficient, outcome-based care.
