Qualifications

• Case Management certification (ACM, CCM, RN-BC) is preferred.
• Basic Life Support Certification (BLS) is required within 60 days of hire.
• 2 years of clinical experience is required.
• Prior case management experience is preferred.
• Quality/Process Improvement experience is preferred.

Responsibilities

• Identify, initiate and manage optimal care coordination and discharge planning to develop patient flow and throughput.
• Collaborate with patient/family, social workers, physicians, interdisciplinary team, and payer to ensure efficient and safe transitions.
• Monitor cost-effective care to ensure treatment in clinically appropriate settings.
• Facilitate compliance with regulatory requirements and organizational performance standards.
• Support assessment, planning, implementation and evaluation of individual patient needs.
