Job summary

HSHS St. Mary's Hospital in Decatur, IL seeks a Care Manager-RN to coordinate patient care and discharge planning. You will work with the care team to ensure efficient transitions, patient safety and optimal utilization of resources.

The role requires an RN license and 2 years of clinical experience, with case management and quality-improvement experience preferred. Day shift, full-time 40 hrs/week, Mon-Fri 8:00am-4:30pm with weekend rotation.

Qualifications

• Associate’s Degree or Diploma in Nursing required.
• Bachelor’s Degree in Nursing preferred.
• Active RN license in state of practice.

Responsibilities

• Coordinate care and discharge planning across the interdisciplinary team.
• Monitor patient progression, safety and length of stay.
• Collaborate with physicians, social workers and payers to optimize outcomes.
