Job summary

Qualifications

• Active RN license in state of practice.
• Experience in health care, care management, or related setting.
• Strong communication and organizational skills.
• Familiarity with EMR systems and HIPAA regulations.

Responsibilities

• Support care management/coordinator activities with multidisciplinary team.
• Develop and implement care coordination plan with members and care network.
• Conduct telephonic, in-person, or home visits as required.
• Monitor and adjust care plans; document interventions and outcomes.
• Maintain ongoing member caseload and outreach.
• Promote integration of services across care domains.
