Qualifications

• At least 2 years in health care, preferably in care management or related field.
• Active RN license in state of practice.
• Valid driver's license and reliable transportation for job travel.
• Knowledge of EMR and HIPAA regulations.
• Ability to work independently and with diverse populations.
• Strong communication and time-management skills.

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Develop and implement care coordination plans with member, caregiver, physician and care team.
• Conduct telephonic, face-to-face or home visits as required.
• Monitor and adjust care plan based on progress and outcomes.
• Maintain ongoing member caseload and regular outreach.
• Promote integration of services across behavioral health, LTSS, and home/community resources.
• Facilitate ICT meetings and team collaboration.
• Educate and motivate members using motivational interviewing.
• Assess barriers to care and coordinate resources.
• Travel 25-40% as required by contract.
• May provide consultation to peers.
