Job summary

Mercy is hiring a Care Manager to support discharge planning from admission to discharge in collaboration with the care team. The role may involve intervening with patients with complex psychosocial needs, ensuring seamless transitions and adherence to standards of care.

Responsibilities include coordinating patient throughput, collaborating across teams, and applying care guidelines to support safe, efficient care and patient outcomes.

Qualifications

• RN with active license in Missouri or state of employment
• BSN required
• 2–3 years acute care hospital experience preferred
• Care Management/Utilization Management experience preferred
• BLS CPR required or within 90 days of hire
• Certification in Case Management preferred

Responsibilities

• Facilitate discharge planning from admission to discharge in collaboration with the healthcare team
• Coordinate patient throughput and care transitions to ensure quality and safety

Job description

The Care Management model provides effective transition planning and length of stay oversight while encompassing quality patient care, patient safety, and financial components; promotes integration of a seamless care model; coordinates patient throughput; encourages healthcare team collaboration and coordination as evidenced by metrics; and optimizes performance and adoption of best practice.

The Care Manager, as part of the Care Management team, facilitates the discharge planning process from admission to discharge in collaboration with the healthcare team. In addition, the Care Manager may intervene with patients who have complex or high-risk psychosocial needs as assigned. Performs duties and responsibilities in a manner consistent with the Mercy mission, values, and Service Standards.

Position Details:

Shift: Saturday thru Tuesday - 10 hour per day shifts.

Education

• Graduate of an accredited School of Nursing, required
• Current License in the state of employment, required
• Bachelor's Degree of Science in Nursing, minimum required

Experience

• 2-3 years acute care hospital setting, preferred
• Care Management or Utilization Management experience, preferred

Certification/Registration

• BLS (CPR) at hire date, Minimum Required, or within 90 days of hire
• Certification in Case Management, Preferred

• Strong verbal and written communication.
• Strong assessment, time management, and critical thinking skills.
• Advanced computer skills.
• Interacts effectively with a variety of people and situations at all levels of the organization.
• The assessment, care, and treatment will be consistent with the specific age-related needs of the patient. Including neonatal/infant, child, adolescent, adult, and geriatric adult patients.
• Knowledge of InterQual or Milliman Care Guidelines preferred.
