Job summary

Qualifications

• Bachelor’s degree in Nursing or related field.
• Four years of RN experience; three years in the area of responsibility.
• Current Tennessee RN license; CCM/ACM/CPHQ certification preferred.

Responsibilities

• Coordinate education of staff and patients and act as clinical resource.
• Develop and implement care management plans with multidisciplinary teams.
• Discharge planning and transition coordination with other facilities and services.
• Participate in quality improvement and regulatory compliance activities.
