Job summary

The Covenant Health Fort Loudoun Medical Center in Lenoir City, TN seeks an RN Care Manager II to integrate evidence-based practice, coordinate staff education and act as a clinical resource for the care team.

The role focuses on care coordination, development of practice guidelines, and quality improvement activities, reporting to the Manager/Coordinator of Quality and Care Management, with a leadership role on the unit.

Qualifications

• Bachelor’s degree in Nursing or related field.
• Four years of experience as a Registered Nurse; minimum three years in area of responsibility.
• Current Tennessee RN license; certification in Case Management (CCM/ACM/CPHQ) or related.

Responsibilities

• Utilizes case management processes to assess and plan patient care.
• Develops and implements practice guidelines and clinical care pathways.
• Educates staff and patients on medications, treatment plans, and discharge instructions.
• Collaborates with physicians and the health care team to plan and implement care.
• Leads quality improvement initiatives and ensures documentation supports patient outcomes.
