Responsibilities

• Uses RN process to guide team toward optimal outcomes and resource utilization.
• Interacts with patients, professionals and the community to ensure continuity of care and document plans of care.
• Conducts comprehensive needs assessments for identified patients and aligns services.
• Develops and maintains accurate case records for referred patients.
• Documents fully; uses appropriate databases and documentation systems.
