Job summary

Phoebe Putney Health System in Georgia is seeking a Nurse Case Manager to oversee care coordination from admission through discharge across PPHS facilities. The CM collaborates with social work, physicians, nurses and the interdisciplinary team to ensure timely, appropriate, and cost-effective plans of care and to optimize patient outcomes.

Under supervision of the Care Management leadership, the CM conducts clinical reviews, uses established criteria, documents in the EMR, and coordinates 24/7

Qualifications

• RN - Registered Nurse license is required.
• Bachelor’s degree in nursing is preferred.
• CCM - Certified Case Manager required within 3 years.
• CHPN - Certified Hospice And Palliative Nurse required within 3 years.
• BCLS - Basic Life Support required.

Responsibilities

• Coordinate care across inpatient, observation and discharge settings.
• Collaborate with social work, physicians, nurses and the interdisciplinary team.
• Document in the EMR timely and accurately and support quality improvement.
