Job summary

Molina Healthcare is seeking a Care Management RN in Wisconsin to support LTSS care coordination and member outcomes. You will collaborate with a multidisciplinary team to develop, implement, and monitor care plans, including waiver services, with regular home visits as required.

Ideal candidates have an active RN license, 2+ years in health care, and strong communication skills. Some states may require a health-care related Bachelor's degree; travel up to 40% may be necessary.

Qualifications

• Requires RN licensure active and unrestricted in state of practice.
• Minimum 2 years health care experience with care management or LTSS preferred.
• Strong communication and ability to work with diverse populations.

Responsibilities

• Complete comprehensive member assessments within regulatory timelines and conduct in-person home visits as needed.
• Coordinate waiver enrollment/disenrollment processes and develop care plans with members and providers.
• Monitor and adjust care plans, document interventions and progress toward goals.
• Facilitate ICT meetings and educate members using motivational interviewing techniques.
