Job summary

Collaboration with patients, families, providers, and payers is essential, with a strong emphasis on improving outcomes and adherence to policy. The role requires current RN license, BSN within four years, and inpatient nursing experience;

Qualifications

• Current Registered Nurse (RN) license in state of practice.
• BSN from an accredited institution; must obtain BSN within four years if hired or promoted.
• Inpatient hospital clinical nursing experience; familiarity with hospital terminology and processes.
• Understanding of disease management, treatment, length of stay, barriers to delivery of care.

Responsibilities

• Identifies patients using screening criteria and conducts comprehensive evaluations.
• Develops, adjusts, and coordinates care plans addressing medical and social needs.
• Guides self-management using motivational interviewing and coaching.
• Provides education to healthcare teams and patients; advocates for patient rights and self-care.
• Works with multidisciplinary teams to ensure timely, high-quality care.
• Leads initiatives to improve care transitions and policy adherence.
• Provides Goals of Care education for informed decision-making.
• Tracks key metrics, analyzes trends, and ensures regulatory compliance.
