Job summary

Brookdale in Phoenix seeks an experienced RN Care Manager to coordinate services across Brookdale HealthPlus communities, promote wellness, and advocate for residents and families.

You will work with primary and specialty care providers to develop resident care plans, monitor outcomes, document meticulously, and participate in collaborative reviews to ensure high-quality outcomes for residents.

Qualifications

• Graduate from an accredited nursing program.
• Minimum 5-7 years of relevant experience.
• Experience working in SNF and HH environments preferred.
• Knowledge of value-based care models preferred.

Responsibilities

• Coordinate services to enhance residents' wellness and manage care needs.
• Collaborate with hospice, home health services and other providers as indicated.
• Provide education to residents and families on care pathways and goals of care.
• Complete required documentation in Brookdale systems and care plans.
