Job summary

Brookdale in Akron, OH is seeking an RN Care Manager to coordinate resident care across teams, families, and providers within our HealthPlus communities. You will develop care plans, ensure services align with diagnosed needs, and advocate for residents while collaborating with hospice, home health, and other partners to improve quality of life.

The role requires an RN license and nursing degree with 5–7 years of relevant experience, in SNF/Home Health settings preferred.

Qualifications

• RN license required.
• Graduate from Accredited Nursing School with 5–7 years of relevant experience.

Responsibilities

• Educates and explains Brookdale’s care pathways based on diagnosis and needs.
• Monitors quality performance measures and trains teams to ensure quality outcomes.
• Reviews residents' medical diagnoses to identify care needs and assess health status.
• Coordinates services to enhance wellness and manage chronic conditions.
• Collaborates with hospice, palliative and home health services as indicated.
• Provides advanced care planning education and ensures completed Advanced Care Plans.
