Job summary

Qualifications

• Bachelor's degree in a related field.
• ACEN/CCNE accredited BSN OR MSN OR Associate/Diploma RN AND 2+ years of experience and BSN/MSN.
• 5+ years of work experience in a related job discipline.
• Active Ohio RN License. May be required to obtain other state licensure.

Responsibilities

• Evaluate timeliness and availability of treatments and services and adjust level of care. Assess patient outcomes versus expected outcomes and identify quality improvement opportunities; report to leadership with supporting data.
• Monitor patient progress toward goals using Clinical Guidelines and Critical Pathways; coordinate tests and procedures with care team; identify barriers to plan and communicate them.
• Facilitate communication across care teams; involve patient and family; coordinate plan of care; ensure smooth transitions; understand regulatory issues and advocate for services.
• Implement plan of care; provide self-management support to high-risk patients and families; educate, empower and problem-solve with patient and caregivers; promote efficient use of resources.
• Plan with patient/family to maximize outcomes; set goals; involve care team; document updates; reassess and adjust plan as needed.
