Responsibilities

• Coordination - facilitate communication across the care team; involve patient, family, and caregivers in decisions.
• Implementation - provide self-management support; educate patients and families about treatment options and care transitions.
• Planning - set goals with patient and care team; document updates and proactively identify needed services.
