Job summary

Lower Keys Medical Center is seeking a Registered Nurse (RN) for Care Management to oversee discharge planning, transitions of care, and case management activities. The role requires collaboration with interdisciplinary teams and ensuring compliance with standards.

The candidate should have 2-4 years of clinical nursing and care management experience, with CM certification (ACM, CCM, or CMGT-BC) preferred. Weekend shifts and student loan contribution are offered, along with full benefits.

Qualifications

• Associate Degree in Nursing required.
• Bachelor's Degree in Nursing preferred.
• 2-4 years of clinical nursing experience in a hospital, home health, or nursing home setting required.
• 2-4 years of care management experience preferred.

Responsibilities

• Coordinate discharge planning and transitions of care.
• Review medical records for appropriateness and medical necessity.
• Collaborate with interdisciplinary teams to optimize patient outcomes.
• Maintain compliance with federal, state, and accreditation standards.
• Develop and implement discharge plans and post-hospital care arrangements.
• Refer cases to physicians/managers when criteria are not met.
• Liaise with community agencies to facilitate smooth transitions.
• Document case management activities accurately and timely.
• Assist patients, families, and physicians with discharge options.
• Attend and contribute to interdisciplinary meetings.
