AdventHealth in Zephyrhills, FL is seeking a Registered Nurse to support discharge planning and care coordination. The role emphasizes multidisciplinary rounds, patient education, and timely disposition planning for inpatient, observation, and emergency department patients.

You will collaborate with families, payors, and post-acute providers to ensure smooth transitions and optimal outcomes while maintaining compliance with CMS discharge planning requirements.

Qualifications

• Associates of Nursing required.
• Bachelor’s degree in Nursing preferred.
• RN licensure required (Florida).
• 2+ medical/hospital nursing experience preferred.

Responsibilities

• Actively participates in multi-disciplinary rounds to review patient status and discharge plans.
• Educates patients and families about illness impact and post-acute resources.
• Assesses readmission reasons for the patient and family.
• Organizes and facilitates patient and family care conferences with the team.
• Documents discharge planning evaluations and progression of care in the medical record.
• Communicates with payors regarding post-acute care authorization as needed.
• Ensures timely discharge planning by considering function, support, and psychosocial needs.
