Job summary

Providence Petaluma Valley Hospital in Petaluma, CA is seeking a Per Diem Care Manager RN for 8-hour Day shifts. The RN will coordinate a continuum of care, establish care plans for a caseload, monitor quality indicators, collect data, document, and report outcomes, while coordinating transitions from acute care to the next level of care.

California RN license is required at hire; 2 years of acute care experience preferred. Bachelor’s degree in nursing and CCM are preferred.

Qualifications

• California RN license required at hire.
• 2 years of acute care clinical or comparable experience.
• Bachelor's degree in nursing; required within 3 years of hire date.
• CCM certification (Upon hire).
• 3 years of experience in community health nursing or case management.

Responsibilities

• Establish a continuum of care plan for a caseload of patients.
• Establish quality monitoring indicators, collect data, analyze, document and report outcomes.
• Liaise with patients to facilitate transitions from acute to next level of care.
