Job summary

Life Care Centers of America’s Valley West is seeking an experienced RN Care Manager on the day shift to coordinate patient care and services across the care continuum. You will work with patients, families, the care team, payers, and external entities to ensure safe transitions and high-quality outcomes in a post-acute setting.

Required qualifications include an ADN/BSN, an active RN license, and at least 1 year of post-acute care experience; CCM or RN-BC certification is preferred.

Qualifications

• Nursing diploma (associate’s or bachelor’s degree in nursing).
• Active RN license in good standing.
• 1 year of clinical post-acute care experience preferred.
• CCM or RN-BC certification preferred.

Responsibilities

• Coordinate the patient’s care and services with the care team, payers, and external entities.
• Promote quality care and safe transitions from the facility to the next care setting.
• Generate written communication that is clear, concise, and well organized.
• Maintain knowledge of laws, regulations, and Life Care standards.
