Job summary

Providence Santa Rosa Memorial Hospital seeks a skilled nurse to oversee patient care from admission, coordinating resources to meet clinical and financial standards. You will communicate daily with medical staff and external resources to ensure optimal resource utilization and patient outcomes.

The role emphasizes risk management, infection control, and collaboration with management. CA RN license is required; BSN preferred and CCM certification is a plus.

Qualifications

• CA RN license required at hire.
• 2 years of acute nursing experience; case management preferred.
• Coursework: Case Management Competency due within 90 days of hire.
• Coursework: General Nurse Competency due within 30 days of hire.
• Bachelor’s Degree in Nursing preferred; BSN within 3 years of hire.
• CCM certification upon hire (preferred).

Responsibilities

• Oversees patient care from admission to ensure availability of clinical and medical center resources.
• Communicates daily with medical staff and external resources to optimize resource utilization.
• Monitors the medical record for variances to clinical/financial standards and intervenes to achieve optimal outcomes.
• Incorporates risk management and infection control knowledge to support the healthcare team.
• Identifies issues exposing the organization to liability and collaborates with management to minimize risk.
• Provides payor/utilization review with concurrent retrospective utilization review for reimbursement.
