Job summary

Orlando Health Melbourne Hospital is seeking a skilled RN Care Manager to promote high-quality patient care and efficient utilization management on an inpatient unit. You will assess needs, coordinate services, and collaborate with the clinical team to optimize patient outcomes.

Responsibilities include leading care conferences, ensuring compliance with Joint Commission standards, and communicating clearly with patients and families about the care plan.

Qualifications

• Graduate of an approved school of nursing.
• Three years of experience in chronic disease management, care coordination, or related field.
• Current Florida RN license.
• Handle with Care (HWC) Certification required within 90 days of hire.

Responsibilities

• Assess patients within assigned population including medical necessity screening and Physician Advisor review.
• Coordinate care that includes admission history, treatments, resources, and discharge planning.
• Lead multidisciplinary patient care conferences and manage complex cases.
• Communicate with patients and families about the plan of care.
• Ensure compliance with regulatory standards including Joint Commission and Medicare/Medicaid requirements.
