Qualifications

• Registered Nurse with current licensure.
• Bachelor's degree in nursing or related field required.
• Minimum 5 years of nursing or 1–2 years in care management.
• Strong communication and interpersonal skills.
• Proficiency with Epic EMR and MS Office.

Responsibilities

• Facilitates communications among patient/family and multidisciplinary teams to resolve barriers to timely care.
• Collaborates with care teams to deliver quality, efficient, and cost-effective services across settings.
• Uses evidence-based approaches to engage patients/families and set SMART goals.
• Identifies barriers to learning and care delivery and develops action plans.
• Manages CM caseload and ensures timely documentation for regulatory and contractual requirements.
