Responsibilities

• Assess, facilitate, plan, advocate, monitor, evaluate, and outcomes per Case Management Society of America Standards.
• Coordinate communication and collaboration among all healthcare professionals involved, with patient and family, to minimize fragmentation of care.
• Lead in changing to improve the quality of patient care and reduce the cost of services on a per-care basis.
• Provide discharge planning services and referrals such as Home Health, DME, SNF and IPR to facilitate timely discharge planning.
• Coordinate care for an average caseload of 20-25 patients per day.
