Job summary

Trinity Health’s Holy Cross Hospital in Fort Lauderdale, FL is seeking a per diem, on-site RN to coordinate and manage patient care across the continuum. The role supports handover, care transitions, and collaboration with interprofessional teams to reduce fragmentation and improve outcomes.

You will ensure accurate documentation, advise on status and resources, and navigate payer requirements while maintaining awareness of reimbursement trends and care quality.

Qualifications

• Bachelor's Degree in nursing preferred with a minimum of 3 years clinical experience.
• Experience: 3-5 years of direct patient care or case management in a hospital setting preferred.
• Acting independently with ability to analyze clinical, financial, and social situations and direct the healthcare team.

Responsibilities

• Ensures appropriate clinical documentation to support level of care.
• Serves as a consultant and educator with medical staff regarding status, payer requirements, and utilization of resources.
• Assures appropriate sequencing of care/interventions for optimal results and smooth transition along hospital care and continuum.
• Proactively identifies and removes barriers that impede the progress of care.
• Ensures clear admission order; meets and acts on cases not meeting admission criteria.
• Maintains current knowledge of healthcare economics, trends, and reimbursement methodologies.
