Ohio's Hospice in Lancaster, OH is seeking an RN Care Manager to plan and deliver patient care using the nursing process. This full-time role serves Lancaster and surrounding areas, coordinating with patients, families and the interdisciplinary team to uphold high standards of professional nursing. The schedule is 40 hours per week, 8:00 a.m.

Qualifications

• Graduate of an accredited nursing school; ASN; BSN preferred.
• Active RN license in Ohio without any board actions.
• Two years acute care nursing experience preferred.
• CHPN certification preferred.
• CPR certified.
• Computer skills sufficient to properly document services and care.
• Able to travel during daytime, nighttime or inclement weather.
• Valid Driver's License with Safe Driving Record.
• State minimum Automobile insurance coverage.

Responsibilities

• Plan and deliver care to patients using the nursing process (assessment, planning, interventions, evaluation).
• Interact effectively with patients, families and the interdisciplinary team while maintaining professional nursing standards.
• Service Lancaster and surrounding areas.
• Schedule: Full-time, 40 hours/week, 8:00 a.m. – 4:30 p.m., occasional holidays and weekend rotation.
