Job summary

Endeavor Health in Arlington Heights, IL is seeking an RN Care Manager to join our patient-centered team. This full-time role, 32 hours per week, positions you Monday–Friday 8:00a–4:30p with rotating weekends to support patient transitions.

You will assess needs, coordinate care plans, and liaise with families and providers while maintaining accurate electronic health records and ensuring HIPAA compliance. A BSN is required; an MSN is preferred; active IL RN license and CCM certification are

Qualifications

  • Bachelor's Degree in Nursing required.
  • Master's Degree in Nursing preferred.
  • 2 years of clinical nursing experience.
  • Current IL RN license required.
  • CCM certification preferred.
  • CPR/BLS certification required.

Responsibilities

  • Assess patients' medical, social, and psychological needs.
  • Develop care plans with the healthcare team.
  • Identify barriers to care and coordinate services.
  • Liaise with patients, families, providers and community resources.
  • Monitor patient progress and manage transitions of care.
  • Coordinate referrals to specialists and home health services.
  • Maintain accurate electronic health records and report outcomes.

Hourly Pay Range:

$40.45 - $62.70 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights
  • Position: RN Care Manager
  • Location: Arlington Heights, IL
  • Full Time: 32 per week
  • Hours: Monday-Friday, 8:00a-4:30p, with rotating weekends
  • 1 winter and 1 summer holiday coverage required
What you will need
  • Bachelors Degree Nursing Required Or
  • Masters Degree Nursing Preferred
  • 2 Years of clinical nursing experience, preferably in an acute care or hospital setting.
  • Registered Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR) Current Registered Nurse (RN) license in the state of Illinois. Required And
  • Certified Case Manager (CCM?) - Commission for Case Manager Certification (CCMC) Certification in Case Management (CCM) or related field Preferred
  • CPR/BLS for the healthcare provider American Heart Association (AHA)
What you will do
  • Conduct thorough assessments of patients' medical, social, and psychological needs.
  • Collaborate with healthcare providers to develop comprehensive care plans tailored to individual patient goals.
  • Identify and address barriers to care and coordinate services accordingly.
  • Serve as a liaison between patients, their families, healthcare providers, and community resources to facilitate the seamless delivery of care.
  • Monitor and manage the progress of patients through the healthcare continuum.
  • Coordinate referrals to specialists, arrange for home healthcare services, and ensure timely appointments and treatments.
  • Oversee transitions of care, including admissions and discharges.
  • Provide patients and families with information about their diagnoses, treatment options, and available support resources.
  • Educate patients on self-management strategies and empower them to actively participate in their care decisions.
  • Maintain accurate and up-to-date electronic health records, documenting all interactions, assessments, and interventions.
  • Prepare and present reports to communicate patient progress and outcomes to the healthcare team.
  • Collaborate with the interdisciplinary team to identify opportunities for improving care processes and patient outcomes.
  • Participate in quality improvement initiatives and contribute to the development of best practices.
  • Ensure compliance with all relevant healthcare regulations and standards, including HIPAA.
