Qualifikationen

• Current, unrestricted RN license in Georgia.
• Nursing degree from an accredited nursing program.
• Experience in home health, case management, supervisory, or geriatric nursing preferred.
• Strong assessment, communication, and organizational skills.
• Ability to recruit, train, supervise, and manage caregivers.
• Professional, compassionate, and dependable.
• In-office and field work; valid driver's license and reliable transportation.

Aufgaben

• Conduct client intake assessments to determine care needs and services.
• Evaluate client needs and develop recommendations for in-home care.
• Communicate with clients, families, healthcare providers, and the care team.
• Ensure client information and documentation are accurate and maintained.
• Monitor client needs and adjust care plans as needed.
• Recruit and interview prospective caregivers.
• Assist with caregiver screening, hiring, and onboarding.
• Provide orientation and training to new caregivers.
• Manage and supervise caregivers assigned to clients.
• Coordinate care and follow up with clients to ensure satisfaction.
• Respond to emergencies and maintain strong client relationships.
