Job summary

Mass General Brigham in Boston seeks an RN Care Facilitator to coordinate patient care in a fast-paced emergency department, using clinical assessment to identify needs and support safe transitions of care.

You will collaborate with ED providers, facilitate transfers, participate in quality improvement, mobilize resources, and ensure timely discharge planning.

Qualifications

• Graduate of an approved school of nursing with MA RN registration.
• BSN required for newly licensed nurses.
• Minimum 5 years acute care experience; emergency nursing preferred.

About This Role

Requirements

Education:

• Graduate of an approved school of nursing with current registration in Massachusetts; BSN required for newly licensed nurses.

Experience:

• 5 years of previous acute care experience required, preferably in emergency nursing.
• 2 or more years of experience in hospital utilization review preferred.
• 5 years experience in an acute setting with discharge planning required.

Licenses & Certifications:

• Current RN License in Massachusetts.
