The Emergency Department Care Facilitator (EDCF) proactively coordinates patient care in a fast-paced emergency department setting utilizing clinical assessment skills to identify patient needs, coordinate care, manage utilization, and support safe transitions of care while supporting patient flow and throughput from the emergency department. The EDCF serves as a key partner to ED physicians, nurses, the interdisciplinary team, and community partners and resources to support timely, appropriate care decisions.
Education: Graduate of an approved school of nursing with current registration in Massachusetts. For newly licensed nurses a Bachelor of Science Degree in Nursing is required.
Knowledge and skills to differentiate levels of care required.
Five years of previous acute care experience required, strongly preferred in emergency nursing.
Two or more years experience in hospital utilization review preferred.
Five years experience in an acute setting with discharge planning and knowledge of community resources required.
Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on their assigned unit.
Previous experience in an acute hospital setting; bilingual preferred; strong clinical assessment skills; excellent interpersonal skills including ability to work collaboratively and cooperatively within a team; strong organizational skills and ability to set priorities; ability to compile data from concurrent and retrospective medical record review to determine discharge plans and create plans to reduce readmissions.
Works within legal, regulatory, accreditation and ethical practice standards relevant to the position and as established by BWH/Partners; follows safe practices required for the position; complies with appropriate BWH and Partners policies and procedures; fulfills any training required by BWH and/or Partners, as appropriate; brings potential matters of non-compliance to the attention of the supervisor or other appropriate hospital staff.
