Qualifications

• Graduate from an accredited school of Professional Nursing.
• Current state RN licensure and BLS certification.
• 3–5 years of nursing experience.

Responsibilities

• Collaborate with multidisciplinary care teams to develop goals and plans for high-risk patients.
• Provide care and disease management coordination and educate patients and families.
• Identify patients needing ongoing coordination and conduct targeted outreach.
• Outline involvement with primary and specialty teams and coordinate care transitions.
• Use assessment tools to identify care needs and manage populations with registries or lists.
• Perform comprehensive clinical assessments across disease-, age-, and treatment-specific needs.
• Advise patients on self-management and monitor progress toward goals.
• Act as liaison and advocate for patients and families across care settings.
