Job summary

Nlc-Support is hiring a Registered Nurse in New Jersey to create and manage individualized care plans for group-home residents. The RN will oversee LPNs, coordinate medical care, monitor progress, and ensure documentation and medication administration are accurate.

The role emphasizes patient advocacy, problem-solving, and effective communication with families and providers. Flexible work-from-home options are available where appropriate, with 8–10 hours on-site weekly.

Qualifications

• Licensed RN in NJ.
• Excellent interpersonal and client care skills.
• Valid driver's license.
• First aid/CPR certification required.

Responsibilities

• Create plans for LPNs to follow to care for the individuals and do check-ins with residents at the group home.
• Provide necessary medical care, monitoring of care, follow-through of care, medication education and other related areas.
• Encourage the normalization and independence of individuals served.
• Provide training and assistance to individuals served and staff on medical education and related areas.
• Act as a liaison to outside medical providers to ensure coordination of care for the individuals served.
• Assist in the administration of medications and required documentation for the individuals served.
• Maintain accurate written records in connection with medical, dental, and related matters for the individuals served.
• Intervene when needed in emergencies with regard to the individuals served.
• Attend interdisciplinary team (IDT) meetings
• Experience working with individuals with developmental disabilities preferred
