Qualifications

• Active RN license in the state of Tennessee is required.
• 3–5 years of clinical or case management experience preferred.
• Strong communication and interpersonal skills; able to build rapport with patients.

Responsibilities

• Guide Care Navigators to support collaborative care management.
• Outreach to patients with chronic conditions to participate in programs.
• Assess and prioritize patient needs and coordinate care plans.
• Review clinical data to monitor adherence to care plans.
• Collaborate with patients and providers to optimize care and close gaps.
• Educate patients on treatment plans, prescriptions, and services.
• Support transitions in care and coordinate with payors as needed.
• Document actions and communicate ongoing needs within the team.
