This role is responsible for delivering holistic care coordination to individuals living with serious or chronic illness through a combination of telephonic, virtual, and in-person interactions.

Brief Description

We have a great opportunity for a RN Care Coordinator (Palliative Care/Principal Care Management). The position offers a hybrid schedule, Monday - Friday 8:00am - 5:00pm. Service area includes Greensboro, High Point, Kernersville, Winston- Salem, Lexington; [Forsyth and Upper Davidson Counties].

The RN Care Coordinator provides high-quality, patient-centered nursing services in alignment with AuthoraCare Collective's mission, values, and clinical standards. This role is responsible for delivering holistic care coordination to individuals living with serious or chronic illness through a combination of telephonic, virtual, and in-person interactions.

The RN serves as a liaison between patients, caregivers, providers, and interdisciplinary teams, ensuring seamless care transitions and addressing medical, emotional, psychosocial, and spiritual needs.

Key functions include clinical triage, care navigation, patient/family education, provider support, data reporting, and workflow improvement. The RN will coordinate care across internal and external providers, contribute to process development, and use data insights to improve outcomes. Conduct focused assessments via phone, virtual platforms, or in-person visits. Develop and monitor care plans in collaboration with providers and patients/caregivers. Provide symptom management, patient education, and psychosocial support. Facilitate and coordinates lab draws, radiology coordination, DME/home health referrals, and follow-up care.

Requirements

Qualifications:

CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.

• Graduate of an accredited Registered Nurse program.
• Current North Carolina RN License required.
• Case Management Certification preferred.
• Minimum two years of RN experience; one year in hospice, acute care, LTC, or home health preferred.
• Strong experience with Microsoft Office, especially Excel.
• Innovative and forward thinking to assist with development of programs.
• Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
